Maternal and Child Health / Healthy Families America is federally funded; Comprehensive Family Support Services are contracts under New Hampshire Department of Health and Human Services, Division of Children Youth and Families; Family Centered Early Supports and Services is federally funded by Part C of IDEA (services for children with developmental disabilities or delays, 0-3). Agencies that operate programs under more than one of these funding sources are also indicated.

Note that this is not a comprehensive list of home visiting providers in the state; rather, it is those known to be providing home visiting through the major federally and state funded contracts. Further, the map represents the site of each of these agencies. In many cases their geographic reach extends beyond their zipcode.
